Resumo

Throughout the twentieth century, in the Spanish mining industry there have been 14 cases of flow failures due to static lique- faction (FFSL) in tailings dams, four of them with catastrophic consequences due to the release of large volumes of water and tailings (TDs), 25 deaths, the affectation of rivers such as Agrio, Guadiamar, Tinto, Besaya, Urumea and large areas of soils due to the release of 4 Mm3 of tailings to the environment. The main triggering mechanisms of FFSL are: failures in the drainage system, overtopping, foundation failure, piping, etc. The failure investigation of TDs after FFSL shows that they have several aspects in common: a) TDs have been built by the hydraulic fill method; b) Tailings are saturated at the time of liquefaction and flow failure; c) The water level in the TDs was higher than prescribed at different times of construction and fill, etc.
